All clients will need to complete a PARQ (Physical Activity Readiness Questionnaire).  A ‘yes’ response to any medical questions, you should consult your GP before you begin personal training.  

It is your responsibility to inform me of any medical conditions which you have.  This may affect your ability to exercise safely or within the correct intensities deemed appropriate for your condition.  

Information should be updated as necessary. You are able to withdraw from the exercise session at any time.  Should you wish to continue, you are liable for any injuries that occur.

Full payment is due in advance, on confirmation of first session date.

Subject to availability, a guaranteed regular diary slot is available for purchase of packs or direct debits.

Short notice cancellations will be charged (minimum 24 hours notice).

Move Naturally Somatics Customer Privacy Notice (this privacy notice tells you what to expect Move Naturally Somatics to do with your personal information).

(a)   We collect or use the following personal information to protect our client welfare:

(i)       Name & Contact Details - to be able to communicate with clients regarding appointments, or any educational material/training plans or advice clients may need to continue their independent training successfully. To submit any invoicing in regard to payment for services.

(ii)      Health and medical information - to ensure Move Naturally Somatics has full knowledge of any medical conditions or health concerns, past or present, that may affect your ability to exercise safely,

(iii)     Emergency Contact Details – Should the need arise to arrange emergency medical care; your Emergency Contact will be notified.

 (b)   Where we get personal information from:

(i)       PARQ - Directly from your completion of the Physical Activity Readiness Questionnaire.

(ii)      Verbally – During any initial consultation process, whether by phone, online or in person.

 (c)   How long we keep your information:

All information held will be kept on file for 5 years from the date you choose to no longer use the services of Move Naturally Somatics.

(i)       PARQ –Your completed questionnaire will be kept on file for 5 years from the date you choose to no longer use our services.  On the understanding that should you require the services at a future date, there will already be a record of medical information, which can be updated if required.

(ii)      Educational material or training plans – Any material relating to knowledge or information provided to you for the purpose of independent training for the reasons as indicated above in item (i).

All of the above also applies if you are a client working with Move Naturally Somatics via zoom or any other virtual communication application.

Lawful bases and data protection rights

 Under UK data protection law, we must have a “lawful basis” for collecting and using your personal information. There is a list of possible lawful bases in the UK GDPR. You can find out more about lawful bases on the ICO’s website.

Which lawful basis we rely on may affect your data protection rights which are in brief set out below. You can find out more about your data protection rights and the exemptions which may apply on the ICO’s website:

 (a)   Your right of access - You have a right to ask for copies of your personal information. You can request details about where personal information is obtained and who it is shared with. There are some exceptions which means you may not receive all the information you ask for.

(b)   Your right to erasure or rectification - You have the right to delete your personal information. - You have the right to ask for correction or deletion of personal information you think is inaccurate or incomplete.

(c)   Your right to restriction of processing - You have the right to ask for a limit on how your personal information is used.

(d)   Your right to object to processing - You have the right to object to processing of your personal data.

(e)   Your right to data portability - You have the right to ask for personal information you gave to be transferred to another organisation.

(f)    Your right to withdraw consent - When we use consent as our lawful basis you have the right to withdraw your consent at any time.

Consent - we have permission from you after we give you all the relevant information. All your data protection rights may apply, except the right to object. To be clear, you do have the right to withdraw your consent at any time

How to Complain

If you have any concerns about our use of your personal data, you can make a complaint using the contact details at the top of this privacy notice. If you remain unhappy with how we’ve used your data after raising a complaint with us, you can also complain to the ICO.
